I the undersigned hereby acknowledges that I am voluntarily participating in Dog Training with Canine Karma and Tina Ferner and her associates (collectively, “Canine Karma”). Canine Karma will exercise all necessary and proper care and precautions to keep all participant and dogs safe during training sessions, classes, workshops, and any and all other activities during the course of my time at Canine Karma ("Activity or Activities") in order to prevent injury to any person, property, other dog or person. However, because dogs are animals and may be unpredictable, Canine Karma cannot protect against every potential injury, risk or accident. This Release and Waiver is intended to shift the risk of participating in any Activity to me as the participant, owner or guardian of the dog participating in the Activity. I understand that my attendance in class is not without risk to myself, my dog, other family members or guests, and other class participants (human or canine). These include, without limitation, risks of physical injury, mental injury, emotional distress, trauma, death, contact with other participants and their dogs, and property damage. All risks are known and appreciated by me. I waive any and all specific notice of the existence of the risks. I hereby accept and assume, without reservation, all risks associated with my participation in any Activity, including, but not limited to: the risks of any and all injuries to myself, my dog and any of my family members or third parties who may attend; the risks that my dog may cause injury to other persons and/or dogs involved; and the risks that saliva, water, food, and/or other debris may be present in the training area. I shall assume responsibility for and pay my and my dog’s medical and emergency expenses in the event of injury, illness, or other incapacity regardless of whether I authorized such expenses. I agree that I, as owner of the above dog(s), am solely responsible for the dog(s) behavior and physical control and agree to carefully monitor and control it at all times.

I further agree that this Release shall be construed in accordance with the laws of the State of Ohio. By signing below, I state that I am fully competent to sign this Release and that I execute this Release for full, adequate and complete consideration fully intending for myself, for my canine and for any family, estate, heirs, administrators, personal representatives, or assigns to be bound by the same. I hereby release and agree to save and hold harmless, Canine Karma, Tina Ferner, any affiliates or property owners from any and all liability, claims, suits, actions, loss, injury or damage of any nature or kind, or for any liability, claims, suits, actions, loss, injury or damage which I or my dog(s) may sustain or which may be caused in any way by my dog(s). I specifically, without limitation, agree to fully indemnify Canine Karma, Tina Ferner, any affiliates or property owners for any and all such liability, claims, suits, actions, losses, injury or damage. I understand that I am solely financially responsible for any damage or harm caused by my dog during any Activity, socializing, or walking.

Some programs are not appropriate for all dogs. Canine Karma reserves the right to remove or refuse a dog from any of the Canine Karma Activities.

CANINE KARMA CLASS RULES

LIST OF REQUIREMENTS AND CONSIDERATIONS WE MUST HAVE FROM ALL OF OUR CLIENTS:

1. VACCINES: We must have a copy of your dog’s most recent vaccines: DHPP, Rabies, Bordatalla (optional) These records can be emailed by you or your vet to: tinaferner@netzero.net . We also must have a copy of a recent (within the last 6 months) negative fecal!

2. WAIVER: We must have a signed copy of our waiver form by anyone who enters our building. If a minor (< 18 years old), must be signed by a parent.

3. PAYMENT AND REFUNDS: There will be NO REFUNDS GIVEN FOR ANY OF OUR CLASSES OR PACKAGES FOR ANY REASON. Prepayment is required to reserve your spot in class. Class size is limited and we will not hold a spot without paperwork and payment. If you must send any records or payment in the mail, please use THIS ADDRESS: Tina Ferner: 2370 Amara Dr, Toledo, OH 43615. *For Preschool Clients Only*: If your puppy’s fecal tests positive after you have registered for class, you may attend class without your puppy to learn all of the skills and then bring pup to class after a negative fecal has been obtained. Another option is to transfer to another class with an additional 50.00 fee which helps us recoup our loss for the last minute cancellation.

4. MAKE UP CLASSES: No make up classes will be given if you are absent for a class. If Canine Karma must cancel a class due to weather or unforeseen circumstances, we will offer a make up class for you. If you wish to schedule a make up class, there will be an additional charge.

5. DOG HANDLER: It usually works best for there to be one primary handler that works with the dog in class. However, other family members or friends are welcome to join the class and are usually beneficial to the socialization process. The handler must be >18 years old. Children between 10-17 years of age must have a parent present with them. Children <10 years old must have an adult with them that is NOT participating in the training sessions. Please see additional restrictions re: COVID and number of participants allowed in class.

6. EQUIPMENT: We use ONLY humane training techniques and equipment. This means NO shock collars, prong collars, choke chains. Approved equipment: buckle collars, harnesses, Gentle leader head halters, and martingale collars. NO retractable leashes! 6 foot leather leashes work well; nylon leashes are OK, but can slip if dog pulls.

7. DOGS AND GREETINGS: Dogs must always be on a leash when in or around our facility.

8. CLEAN UP AFTER YOUR PUP! We work hard at maintaining a clean training environment. Please join us in doing all we can to prevent accidents and always clean up after your pup!

9. CLASSES START PROMPTLY AT TIME SCHEDULED: Classes start on time. Please arrive early to potty your dog prior to class. Please use the grassy area across from the lot and NOT the area directly in front of our door (per management). Please bring your own poop bags to use in case it is needed and deposit waste in can in front of our door. Feel free to excuse yourself during class if you think your dog needs “to go”. There will also be reminders in class for a potty break.

10. CONSENT FOR PHOTO OR VIDEO: We occasionally take photos or short video clips of our classes as well as our students upon graduation. These MAY be posted on Facebook or used for illustration for future purposes. By signing below, you give Canine Karma consent to do this.

11.PERMISSION TO CONTACT YOUR VETERINARIAN: By signing below you give Canine Karma permission to contact your veterinarian to share information about your dog or to provide a “progress report”.

12.IF YOUR DOG IS ILL: If he/she has vomited or had diarrhea 48 hours prior to your appointment day, your dog should not come to class.
