Negative Fecal Test *Upload a photo or document of proof from your veterinarian that your dog has produced a negative fecal test in the last 6 months. (Must be png, jpeg, gif, or pdf)
Recent Vaccine Records *We must have a copy of your dog’s most recent vaccines: DHPP, Rabies, Bordatalla. Upload a photo or document. (Must be png, jpeg, gif, or pdf)
I have read and agree to the terms included in the Canine Karma Liability form, Class Rules, and Responsible Dog Owner's Pledge *Sign your first and last name

Please save a place for me in the October class. I pick up my puppy Monday in Maine. Today was the litters vet visit with shots and microchip. I will receive the information when I pick her up and can finish registration on Tuesday.

I meant to sign up for the Sunday January 7th 2024 class. On the date portion of the registration for it auto filled and I thought that was what you were wanting.

I am trying to fill out STAR class form and submit for beginning Sunday class (08/24/23) for Ella Bleu. I submitted vet forms photo. I hope that works for this submission. I can additionally have west suburban fax and relative updated health and/or vaccine records. The fecal test shows completion in 06/15/23 and was negative.

We do not yet have Coco’s medical papers from the breeder, and she has not had her first visit with her new vet. We would like to attend the Sunday class that begins on November 5, 2023. I just wanted to register to be sure we got a spot in the class. Give me a call and let me know if this is ok or if I should wait to register until we have the medical needs met.

I will get a new fecal and vax record closer to class time. Looking forward to getting back with you and moving forward with the rest of the courses you have. Henry has such potential I need to explore it.

Roscoe and I would like to register for the next available STAR class. Looks like that begins on January 8, 2023, Sunday, 9am.

We filled out the questionnaire and uploaded vet info.
